Health Forms are required to be on file for any youth participating in Munsey Youth activities. These Health Forms are good for one year and must be notarized. No youth will be allowed to participate in an offsite event unless we have a form.Download a health form.
If you are under age 18, a Permission Slip is needed for all out of town trips and in-town activities where you might get hurt, such as go-carting, slip-n-slide and water sports . A new Permission Slip is needed for each new activity. Download a permission slip here.
Forms. The forms we use most often are the Health Form and the Permission Form. The Health Form (tan) is needed when youth travel on church trips or do something in town that may put them at risk for injury or harm. It is 2-sided, needs to be signed by parents, and should be accompanied by a copy of the insurance card. The Health Form can be placed on file in the Youth Ministries office if it is notarized – and it is good for one year from the date of notarization. For one-time visits, notarization is preferred for Health Forms, but it’s okay not to have it notarized if that proves too difficult.
Likewise, the Permission Form (yellow) is needed when youth travel on church trips out of town or do something in town that may put them at risk for injury or harm. The Permission Form lets us know that parents know what their youth is doing. Youth should give forms to the adult leaders at the beginning of an event. Forms are available online at www.munsey.org or at various locations throughout the church – the Connection Point, outside Dan’s office, and the info table at the entrance of the youth floor.
